The first portion of a work trial agreement must contain the basic details of the employee and the host employer details. Such details are the names, address and company name.

If the host employer, and trainee wish to alter, extend or close this work trial, prior to expected completion date, this should be done in advanced and is expected to be discussed between the trainee and employer to reach an arrangement.

Indicate the work trial position details, the days advised to work and the number of hours that should be rendered. Any medical restrictions or other important considerations must also be implied in the agreement. Work schedule must contain the employee’s duties/task, considerations and hours rendered.
IV. Work Trial Terms and Conditions
The terms and conditions of the work trial must be thoroughly pointed out in the agreement. Such information are as follows:
- Payment agreement: if the work trial is paid, then it should be discussed how often should the trainee should be paid. If unpaid, then this should be also indicated.
- Work related expenses should be shoulder by the employer
- Since the employee is still a trainee, then any company related benefits and compensation is not applicable unless he or she has become a regular employee.
- Work related incidents resulting to injury should be covered in a clause in the agreement as to what extent can the company cover such expenses.
- Agreeing to participate in this work trial does not imply employment or another contractual relationship between the trainee and the host.
V. Work Trial Host Employer Declaration
In this section specify the different responsibilities to be shouldered by the host employer, with the name and signature of the host employer agreeing to the terms.
VI. Work Trial Trainee Declaration
