What Is a Doctor’s Report?
A doctor’s report, commonly known as a physician report or medical report, is a written statement that contains diagnoses, results, and other pertinent medical data that are relevant to a medical examination. The doctor’s report can be issued by healthcare physicians such as surgeons and medical specialists to explain the clinical findings of a patient including the provided and proposed treatments. This report also contains the patient’s medical history to give the readers more insight into a patient’s condition.
How to Make a Doctor’s Report?
A doctor’s report is one of the most complicated documents in history as it needs a medical professional to be able to complete it. If you are just starting with your journey in the medical field, we can help you out in making your very own doctor’s notes by following the basic steps we have provided for you below.
1. Patient Information
The first thing you need to write down in the medical report is patient information. Include their full name (aliases if they have no identity, to begin with), age (or estimated age), and gender. Also indicate whether the patient is alive or already deceased (in the case of autopsy). If this is a standard medical report for physical examinations, you can even include their contact information just in case.
2. Medical History
Has the patient undergone previous medical treatment or procedures? Does the patient have a prior medical history that is related to the case you are handling today? If so, you need to write that down for references. Knowing what the condition of the patient was before being examined by you or admitted to the hospital makes it easier to provide accurate medical treatment and suggest procedures to cure and save them before symptoms worsen.
3. Laboratory Examinations
If the patient has undergone laboratory examinations, it is required that you include them in the report. Write down blood tests, urinalysis, fecal exams, and other relevant examination results to understand the condition of the patients and to provide the correct prognosis for it. If you decide to have the patient take a repeat lab examination, then the current result would be the best reference
4. Patient Treatment and Progress
Last but not least, one of the most important components that you’d need to include in your report is the patient’s treatment information and the progress they are making after the treatments. What medications have been prescribed? How many days after a medical procedure did the patient start getting better? Are they receptive to the treatment or are displaying worst symptoms than before? Include this information as well to complete your medical report.
5. Medical Opinion
After examining the patient, do you have any suggestions or medical opinions that you wish to share with the patient and their family? If so, write it down on the bottom part of the report as this will serve as your notes on which the patient and their kin would consider or not. As a medical examiner, write your opinion as simply as you can so that the patient can understand what you are trying to say. Remember, not most people do understand medical terms, especially ones that they just have heard about.
Why are medical reports important?
Medical reports are important as they help healthcare professionals provide the right care and treatment to a patient by using them as medical references. It would also provide deeper insight to the patient’s conditions as well as become a material that may help in clinical studies in the future.
Who is allowed to write medical reports?
Only professional healthcare providers are allowed to make medical reports. These include doctors, health specialists, dentists, nurses, or post-grad medical students.
