What Kind of Notes Are Inappropriate in a Patient Chart?

Patient notes are similar to nurses notes and doctor notes in word. It is important that chart notes are written professionally.

  • Subjective notes telling about how something that should have been done and using accusing or faultfinding words are very inappropriate to write in a patient’s chart.
  • Criticism about the care provided by other nurses or other health providers should not be added in the patient’s chart.
  • Never write insults in reference to someone.
  • Do not copy care notes that have been provided by another provider.

What Is the Purpose of Patient Notes?

  • A patient’s notes contains all of their hospital records including those records from their previous hospitals visits as well as those from their previous admissions.
  • It includes a patient’s health history, general information, past and present illness, the care that they have received, the progress that they are undergoing, and so on.

Tips and Advice About Writing Patient Notes

Be able to know what to do and what to write in a patient’s notes. They are like handover notes that health providers of the next shift use as reference of the care that was provided. Here are some helpful tips that you can use.

  • Do not leave empty spaces or blank spaces in between your notes. This is not only done to save space,it is  also to make sure that nobody writes something in between those lines that may get you in trouble. You are responsible for anything that is written within your notes.
  • Never make any erasures. This can be taken against you and your care may be considered questionable. Too much erroneous charting may cause you or the hospital to end its operations. If you really have to, follow the proper procedure to make corrections.
  • Avoid writing comments or suggesting anything rude about a doctor’s or health providers notes.
  • Never write any subjective findings or descriptions in your notes.
  • Always make sure that you have the right patient’s notes on hand before you start writing anything.
  • Never mention the lack of staff in notes.
  • Incident reports should be nonexistent in a patient’s chart.
  • Do not use insulting words to describe your patient while charting.
