Applying for a job can be daunting. Whatever the nature of the position, there will always be a set of qualifications an applicant will have to meet, requirements to comply, and procedures to undergo. One of these procedures is a background check. It is a standard preemployment procedure wherein a job applicant’s background is scrutinized to ensure that they do not have any criminal records and are suitable for the position.

A background check requires the consent of the individual whose background is to be probed. The Sample Forms on this page show the various formats of background check release forms that are used to inform an individual of the procedure, as well as obtain their permission.

Background checks are done in different ways. The procedure followed is dependent on the employer or the company. Commonly, the recruitment department of an organization will look into the employment history of an applicant, as well as whether or not they have any criminal records. The person who conducts the background check will communicate with the applicant’s past employers, asking for insights and opinions regarding the applicant’s character and competence. They will also contact people in law enforcement to check if the applicant has had any run-ins with the law.

Prior to this procedure, the human resource personnel will inform the job applicant that they will conduct a background check. They usually convey this to the applicant through Background Check Forms. These forms are legal in nature, and other than informing the applicant, they also serve to obtain consent from the applicant by having them fill out and sign the form.

These forms are much like Medical Release Forms. However, they differ in terms of context and purpose. While background check forms serve to probe the employment history and criminal records of an applicant, medical release forms are required for a practitioner to be able to access an individual’s medical history.

A release form is a legal document that does not attest full compliance of the releasee, but only their permission to allow the releasor to execute what is stated on the form and nothing more. There are different types of release forms apart from background check release forms; Photo Copyright Release Forms and other copyright documents are especially common nowadays.

Like background check release forms and medical release forms, photo copyright release forms contain the terms suggested by the releasor. In this type of release form, the authors of original work such as film, photos, and audio files are commonly the releasor. The releasee, on the other hand, can be the talent or staff, location owners or managers, or even clients. These forms serve to protect the releasor from liability from those who partook in the production of their work when they decide to publish or showcase it.

In the context of background check release forms, the releasor is the company or the human resource personnel representing the company. The releasee is the job applicant. Commonly, the party conducting the background check will state on the form the means they will follow in conducting the procedure. As seen on the samples on this page, most documents will have space for the releasee’s signature and also for the date when they signed it.
